Care Prioritizer Portal
Creating software for quality improvements in healthcare
Healthcare data and population management is one of the biggest concerns facing providers today. I worked closely with a provider team in Florida to create a managed care system that would replace their current one that had been recently sunsetted.
One of the biggest challenges was around requirements gathering. Their initial ask was to replace what they had. This lead to a lot of questions, what was it that worked well, what didn’t work, what was your workflow, where did you spend most of your time, what data was most important.
Combining this with what our current platform was capable we came to the conclusion this would need to be separate platform.
I started off with a data dive. Looking at all the data that was collected, from socio-demographic, to prescriptions and health history revealed several layers. The client requested a methodology for prioritizing utilizing insights and alerts.
The result came out to be a platform that enabled multiple ways to engage the clinical staff, with windows allowing for views into their priorities without distracting from their current objective. Each new patient should be one click from the patient they are on. Contact info is easily accessible and their history spelled at at a glance.
